Registering a birth: information needed

Children

  • Date and place of birth (if it was a multiple birth then the time of each birth is also needed)
  • Sex of the child
  • The forename(s) and surname in which it is intended that the child will be brought up
  • The Personal Child Health Record, also known as the Red Book

You should also take supporting documents that show your name and address (eg passport, driving licence, utility bill), you can however still register a birth without them. The registrar will use these documents to improve the quality of the information recorded in the registration.

Father (where this information is to be entered in the register)

  • Forename(s) and surname
  • Date and place of birth
  • Occupation or if not employed at the time a previous occupation

Mother

  • Forename(s) and surname
  • Maiden name if the mother is or has been married
  • Date and place of birth
  • Occupation if not employed at the time a previous occupation
  • Address at the time of the birth
  • Date of marriage if married to the child’s father
  • Number of previous children
